Polk Street Senior Housing opened its doors in November 2008 to house formerly homeless and low-income seniors in San Francisco, many with complex medical, mental health, and/or substance use concerns. Our community is home to just over 50 formerly homeless seniors who haved transitioned into independent living from homeless shelters, medical and psychiatric programs, substance use treatment programs, and transitional living situations. Full description


Forensic Housing Program

Lutheran Social Services’ Forensic Housing Program (FHP) is a collaboration between LSS and direct referrals from city and county programs including the Forensic AIDS Project, CenterForce, the San Francisco AIDS Foundation, Mission Neighborhood Health Center, the South East Health Center, Alliance Health Project, UCSF Ward 86, the Tenderloin Area COE, and the San Francisco Pretrial Diversion Project.

Stabilization Program

Lutheran Social Services of Northern California (LSS) — Stabilization  Housing Program  provides  hotel rooms and client-centered on-site services and case coordination to medically stable homeless individuals who are HIV+ with the goal of stabilizing clients’ health and increasing housing stability.
