The Building Bridges Program provides six months or longer of case management and supportive services to previously homeless individuals and families who graduate from one of Sacramento's transitional housing programs for homeless persons. The Program seeks to prevent a return to homelessness by fostering the participant's personal and economic self-sufficiency through strengthening the participant's own system of support and assistance, community referral, advocacy, and financial assistance for security deposits, essential furnishings, and educational/employment-related expenses.

Program Goal:

To provide permanent supportive housing to both individuals and families in Sacramento County.

Who Is Served

Families/individuals qualify for the program if they:

• Have successfully completed/graduated from a Sacramento transitional housing program.
   
• Have demonstrated a willingness to accept assistance with their move toward independence.
   
• If there is a history of drug and or alcohol addiction the participant must be in active recovery for at least six months prior to referral and must continue recovery while in the program.
